Authors should clearly define and distinguish between sex and gender in their research:
Sex refers to biological attributes (chromosomes, hormones, reproductive organs) and is generally categorized as female, male, or intersex.
Gender refers to socially constructed roles, behaviors, expressions, and identities of girls, women, boys, men, and gender diverse people.
These concepts are distinct but interrelated, and clarity in their use is essential for accurate interpretation and application of research findings. When either term is used, authors should clearly specify which construct they are referring to and justify their choice based on the research context.

Authors should:
- Conduct a thorough review of existing literature on sex and gender differences relevant to their research topic
- Synthesize current knowledge about biological and social factors that may influence outcomes

• Consider sex and gender in sampling strategies to ensure sufficient statistical power

• Select appropriate measures and instruments that are validated for diverse populations
• Document and report the sex and gender composition of study samples

Unlocking Equity: The History of SAGER Guidelines
Gender Policy Committee
As an elected EASE Council member, Dr Shirin Heidari founded the Gender Policy Committee to strengthen sex and gender considerations in research.
Global Consultation
Co-chaired by Dr Shirin Heidari and Dr Thomas Baber, the EASE Gender Policy Committee developed the draft SAGER guidelines and led an international consultation.
SAGER Guidelines Launch
The SAGER guidelines were published, establishing an international framework for systematic sex and gender reporting across research disciplines.
Global Promotion
The EASE Gender Policy Committee advanced the promotion of the SAGER guidelines, while editorial societies supported translations to enable wider international use.
GENDRO as Custodian
Following the conclusion of the EASE Gender Policy Committee’s mandate in 2023, GENDRO became the custodian of the SAGER guidelines, working with EASE and partners to advance their use.
SAGER-Ethics Working Group Established
